All outside agencies are welcome to send referrals to Guardians to request placement for their clients. Guardian staff will review the referral and notify the requesting agency within 24 hours after submission of the referral packet to verify receipt of information. If additional information is required, the requesting agency will be notified. Upon review, the requesting agency will be notified 24 to 48 hours of our intent to treat or a written denial of services. If an individual is found to have behavioral health needs beyond our scope of services; Guardians of the Four Corners will not accept this client.
Clients can reach out to Guardians independently, but it is highly recommended the client have previous records for staff to review. The staff will review each case on an individual basis and determine if admission is appropriate for both the client and the facility.
A client will be voluntarily discharged when they have achieved their treatment goals as documented in the client’s treatment plan and can demonstrate the desired skills as mutually determined by the client, legal guardian or representative, and the Guardians Administrator or his/her designee. At the time of discharge or transfer a client will receive a referral for treatment or ancillary services if determined as necessary by the Clinical Director.
Involuntary Discharge:
This will occur when the client’s behavioral health issues or treatment needs are not consistent with the services our agency is authorized to provide. Clients who are unwilling to engage in treatment and/or program policies may also be discharged. This may include, but is not limited to: verbal/physical assault/abuse of staff or other clients, or disruptive behaviors that interfere with the treatment needs of other clients in the program
