Guardians of the Four Corners welcomes Male & female adults 18 to 62 years of age. Our clients may exhibit one or more of the following:
Attention Deficit Disorder/Hyperactivity Disorder
Personality Disorders
Depression
Bipolar
Anxiety
Self Destructive Behaviors
PTSD/Trauma
Substance Use Disorders
Dual Diagnosis
Admission Criteria
Client has a diagnosed behavioral health condition
The behavioral health condition causes significant functional and/or psychosocial impairment by evidence by:
At least one area of significant risk of harm.
Impulsivity with poor judgment/insight
Maladaptive physical or sexual behavior
Inability to remain safe within environment
Medication side effects due to toxicity or contraindications.
At least one area of serious functional impairment as evidenced by:
Inability to complete developmentally appropriate self-care or self-regulation
Neglect or disruption of ability to attend to majority of basic needs, such as personal safety, hygiene, nutrition, or medical care,
Frequent inpatient psychiatric admissions, or legal involvement due to lack of insight or judgment associated with psychiatric disorders,
Frequent withdrawal management services, which can include but are not limited to, detox facilities, MAT, and ambulatory detox,
Inability to independently self-administer medically necessary psychotropic medications despite interventions
Impairments persisting in the absence of situational stressors that delay recovery from the presenting problem.
Has a need for 24 hour behavioral health care and supervision to develop adequate & effective coping skills that will allow the member to live safely in the community.
Anticipated stabilization cannot be achieved in a less restrictive setting
Evidence that appropriate treatment in a less restrictive environment has not been successful or is not available,
(IOP, supportive services have not been successful)
**Clients will be served on a case-by-case basis as determined medically necessary by the Administrator of Guardians of the Four Corners.
All adults referred for services must meet the basic requirement of being unable to be successful in a less restrictive setting due to negative behavioral outbursts, impulsive and destructive behaviors, and a diagnosed behavioral health disorder, family conflict, or incorrigibility.**
